Compassionate Care

At iServe, we are dedicated to providing comprehensive hospice care services throughout Dallas-Fort Worth that is tailored to the unique needs of our patients, whether it’s specialized therapy, personal care assistance, private-duty nursing, or home dialysis. To be able to provide this high level of care, we have a team of healthcare professionals that are well-experienced in dealing with patients and their loved ones.

Our Hospice Care Team

Our patient-first team prioritizes comfort and health to ensure the highest quality end of life care is provided anytime and anywhere. Our experienced team includes:

Physician

Our physicians are responsible for providing orders for hospice care. Along with our nurses and other members of the care team, they will create a care plan that is tailored to the unique needs of each patient based on the initial evaluation, patient needs, and official diagnosis.

Nurse

As we’re owned by nurses, the nursing staff at iServe Hospice are experienced in carrying out our weekly evaluations and reports shared amongst the care team. Our nurses focus on comfort and quality of life to meet the physical, emotional, and spiritual needs of our patients.

Nurse’s Aide

Our nursing aides help patients with activities of daily living (ADL) like bathing, feeding, transfers, changing linen, and incontinent care.

Social Worker

Social workers play an important role in assessing our patients’ and their families’ psychosocial needs. They can also support families by helping them connect with the right community resources.

Chaplain

The emotional and spiritual needs of our patients are just as important as the physical ones, and our chaplain can help support the spiritual needs of our patients. This care will often include spiritual and emotional counseling or just providing an open ear and comfort.

Volunteer

Our volunteers are special individuals who have a genuine love for caring for our patients who need companionship and friendship. Volunteers also provide assistance with care team tasks while supporting the family.
