The Hospice Team
An advanced home care support team is provided to the patient and their family.

- Responsible for providing orders for hospice care. Ensures the care plan meets criteria based on the patient’s care and diagnosis.

- The nurse visits weekly for patient evaluation and reports to the interdisciplinary team for any change in patient condition.
- NURSE’S AIDE
- Completes visits according to the patient’s needs to assist with activities of daily living (ADL) such as bathing, transfers, incontinent care, feeding, ambulation, and linen change.
- SOCIAL WORKER
- Assess the patient and family for psychosocial needs, and assist with providing community resources to improve the quality of the patient and their family.
- CHAPLAIN
- Assess the patient and family for spiritual needs, and assist with providing spiritual and emotional counseling to improve the quality of life of the patient and their family.
- VOLUNTEER
- Visits the patient who needs companionship and friendship at home while assisting with tasks and supports the family may need.
