What is hospice care?
Hospice care occurs at the patient’s home or place of residence after a physician determines that the patient has a terminal diagnosis or has six months or less to live. The focus for hospice is to keep the patient comfortable to reduce aggressive symptoms.
What services do iServe hospice provides
Management of the patient’s pain and symptoms;
Assessment and assistance with patient and family spiritual and psychosocial needs;
Provides medical supplies, equipment, and medication management;
Educate the family on patient care;
Provides therapy services as needed, including speech therapy, physical therapy, occupational therapy, and nutrition needs;
Provides short-term respite care for burned-out caregivers, continuous care for patients in crisis, and provision of a general inpatient unit for patients who may need continuous intravenous fluids and other injections.
Provides 12 months of bereavement care for the family who lost their loved one.
Who are the members of the hospice team?
The interdisciplinary group (IDG) usually consists of:
The patient’s physician;
Hospice physician (or medical director);
Nurses;
Hospice aides;
Social workers;
Spiritual care providers or other counselors;
Bereavement professionals;
Speech, physical, and other occupational therapists; and
Trained volunteers.
What duties do hospice volunteers perform?
What duties do hospice volunteers perform? Hospice Volunteer assists patients and their families in relieving caregivers, bathing the patient, and doing other household chores. They may also serve as companions to patients who do not have family members. Volunteers also assist in the hospice office management.
How does hospice care work?
Most patients in hospice care make decisions by themselves. A patient does not have to be home-bound to be in hospice. A hospice patient can visit relatives, attend church, and engage in other activities. A primary caregiver appointed by the family or medical power of attorney may represent patients who cannot make decisions independently. With a care plan, the patient will receive services from the hospice team, including the hospice medical director, RN who visits weekly, hospice chaplain, hospice medical social worker, hospice aide, and volunteers.
Who should receive hospice care?
Hospice care is for a patient whose doctor has determined a terminal diagnosis and has six months or less to live. A registered nurse and hospice medical director must carefully assess the patient’s diagnosis and symptoms to make a definitive certification for terminal illness. Typical diagnoses for hospice care include cancer, dementia, Alzheimer’s, COPD, heart failure, liver failure, and kidney disease. These diagnoses, in most cases, should accompany total care, decrease endurance to physical activity, weight loss, frequent hospitalization, frequent UTI occurrence, and falls.
Who covers hospice care cost?
Medicare pays for all hospice services, and Medicaid covers room and board for patients that live in a nursing home. Hospice provides all medical equipment and supplies that the patient needs throughout hospice care. Families who do not have Medicare may pay out of pocket or use private insurance.
Should I be homebound to qualify for hospice?
Unlike home health, you do not have to be homebound to be in hospice. You can be in hospice and still have the leisure to visit relatives, go to church, and engage in other recreational activities if you have the strength.
